I actually find Sera Myu to be a lot more confusing. After that, it would be the manga (which I had to read some acts twice), then PGSM, then the anime.
With PGSM, you have to pay attention. I never thought it very confusing. If you watch everything in order, and don't start with Act Zero, I don't think you should have that much of a problem.
Then again, it could just be me.
But if you want something really confusing, try to figure out the Sailor Moon timelines.

I didn't find the PGSM story to be CONFUSING really... it was just... so choppy, so occasionally it does cause confusion... It's the way of Japan lol Other than anime... Pretty much every Japanese movie/television show I've seen has has a lot of jumps and weird confusing choppy editing... I think it is just the way they do things.
Believe me, once you start watching a lot of them you start feeling the pattern, then it doesn't get so confusing anymore. You just have to get used to the Japanese way of editing/storylining as far as live action. Go watch a few Japanese horror films from your local movie place and you'll start to feel it (It'll be SOO confusing at first lol believe me my first few of those totally confused me... but once you watch 5 or 6 of them you kind of understand, and then all things like that start to make a lot more sense...)
I think it's just a cultural difference.... it seems to me anime is almost made in an AMERICAN way (although totally a Japanese idea... if you understand what I mean lol) and yet the shows/movies are made completely in a Japanese way...
If that makes sense...

I didn't find PGSM very confusing, but that could just be the way I work. But yeah, don't start with Act Zero or you will be very confused. Or the Special Act. You might need to concentrate, you might not, because I'm not sure what to honestly say -- all I know is that I watched it all in 24 hours, and then I watched Act Zero afterwards via YouTube.

As long as you have a vague understanding of the manga and anime plots, I think you'll get it! Some of the plot lines are a bit hard to follow, and the outros and intros tend to be a bit confusing sometimes. Is there something in particular you don't understand, Moon Light Real Girl? The 'new' characters threw me a bit to begin with. all I know is that I watched it all in 24 hours, and then I watched Act Zero afterwards via YouTube. Woah.

Act 47 threw me for a loop, because the opening clips was exactly the same as a clip shown later in the episode...

The story line was clear, for me... The only thing I found confusing was the characters, at first they all seemed similar to me, especially the girls, but after two episodes it all ended up and I started enjoy everything. X)

I didn't find it confusing at all though I did have a hard time wrapping my brain around Princess Serenity-as-an-antagonist for a bit cause in the anime and manga she's presented as an absolute force for all that is good and pure. After the initial "WTF!?" I did like what they decided to do with the character.

I didn't find PGSM confusing at all. I started with Act 1, watched my way through the series, then watched Act 0 last. I still haven't watched the Special Act... heh.
Honestly, I found the manga a little more confusing than PGSM, but that's just me!

PGSM didn't really confuse me. I mean, if you skip a few episodes, then yes, you'll be completely lost. I watched the show in it's proper order, though.
The manga confused me at times, though. It's because there's SO MUCH in there that I had to review stuff while reading it for the first time.

I can see why people might get confused by PGSM, especially if you're already familiar with the other versions. Compared to them, who's on which side and what the sides are are much less clear-cut, and keep changing.... the same person who wants to kill you in one episode will die to save you in another.... I've generally been able to follow everything, but I do remember one episode, can't remember which number, when Jadeite started fighting Kunzite and I was like "wtf? I thought you guys were on the same side at this point in time...." EDIT NEARLY A YEAR LATER: Well, I've finally gotten round to seeing the series a second time, and I think it's made it much easier; knowing from the start how the storyline goes made it easier to understand the characters' motivations and what sides they're all on. The bit I referred to with Jadeite fighting Kunzite was of course Act 35, and yes, I understand it now
